et de mortalit\u00e9.<\/p>\n<p>D&rsquo;o\u00f9 la n\u00e9cessit\u00e9 d&rsquo;une stratification du risque cardiovasculaire, selon des facteurs li\u00e9s au patient (cardiopathies actives, facteurs de risque clinique, capacit\u00e9 fonctionnelle) et \u00e0 la chirurgie aria-labelledby=\"elementor-tab-title-2101\"><p>Les anticoagulants peuvent \u00eatre utiles pour r\u00e9duire le risque thromboembolique (TE) associ\u00e9 \u00e0 une fibrillation ou un flutter atrial\u00a0<a href=\"https:\/\/www.e-cardiogram.com\/fibrillation-atriale-7b-anticoagulant\/#_ftn1\" name=\"_ftnref1\">[1]<\/a>\u00a0.<\/p>\n<ul>\n<li>Le risque estim\u00e9 selon le score CHA2DS2-VASc varie de 0 \u00e0 9 points et pr\u00e9dit une incidence annuelle d\u2019AVC variable entre &lt; 1 % et &gt; 20 % (respectivement 0,8 ; 2,0 ; 3,7 ; 5,9 ; 9,3 ; 15,3 ; 19,7 ; 21,5 ; 22,4 ; 23,6 %) [2]. Soit x2 jusque 4 puis x3 de 5 \u00e0 9 (environ)<\/li>\n<li>Il augmente pour une FA de dur\u00e9e &gt; 48 heures et apr\u00e8s cardioversion (entre le 3e et le 30e jour).<\/li>\n<\/ul>\n<p>Tous les patients qui pr\u00e9sentent une FA doivent avoir une estimation du risque thromboembolique (TE), quel que soit le rythme cardiaque au moment de l\u2019examen (ex. conversion ou cardioversion). L\u2019estimation du risque TE est essentielle \u00e0 la prise en charge th\u00e9rapeutique. Elle repose sur le score CHA2DS2-VASc (1). Pour simplifier, le risque TE la premi\u00e8re ann\u00e9e de d\u00e9couverte d\u2019une FA est x 2 de 1 \u00e0 4 puis x 3 au del\u00e0.<\/p>\n<ul>\n<li>Les facteurs de haut risque d\u2019accident thromboembolique sont : un \u00e2ge sup\u00e9rieur \u00e0 75 ans, un ant\u00e9c\u00e9dent d\u2019accident vasculaire c\u00e9r\u00e9bral (AVC), accident isch\u00e9mique transitoire (AIT) ou embolie syst\u00e9mique\u00a0<a href=\"https:\/\/www.e-cardiogram.com\/fibrillation-atriale-7b-anticoagulant\/#_ftn1\" name=\"_ftnref1\">[1]<\/a>, un r\u00e9tr\u00e9cissement mitral ou la pr\u00e9sence d\u2019une proth\u00e8se valvulaire cardiaque m\u00e9canique\u00a0<a href=\"https:\/\/www.e-cardiogram.com\/fibrillation-atriale-7b-anticoagulant\/#_ftn3\" name=\"_ftnref3\">[3]<\/a>.<\/li>\n<li>Un traitement anticoagulant au long court par anticoagulant oral est recommand\u00e9 en pr\u00e9sence d\u2019un facteur de risque thrombo-embolique (grade 1A), \u00e0 l\u2019exception du sexe f\u00e9minin.<\/li>\n<\/ul><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-toggle-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-2102\" class=\"elementor-tab-title\" data-tab=\"2\" role=\"button\" aria-controls=\"elementor-tab-content-2102\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-toggle-icon elementor-toggle-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-toggle-icon-closed\"><i class=\"fas fa-caret-right\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-toggle-icon-opened\"><i class=\"elementor-toggle-icon-opened fas fa-caret-up\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-toggle-title\" tabindex=\"0\">Score HAS-BLED (risque h\u00e9morragique)<\/a>\n\t\t\t\t\t<\/div>\n\n\t\t\t\t\t<div id=\"elementor-tab-content-2102\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"2\" role=\"region\" aria-labelledby=\"elementor-tab-title-2102\"><p><span style=\"font-size: 12pt;\">Tous les patients pr\u00e9sentant une FA doivent avoir une \u00e9valuation formelle du risque h\u00e9morragique (ex. score HAS-BLED\u00a0<a href=\"https:\/\/www.e-cardiogram.com\/fibrillation-atriale-7b-anticoagulant\/#_ftn1\" name=\"_ftnref1\">[1]<\/a><a href=\"https:\/\/www.e-cardiogram.com\/fibrillation-atriale-7b-anticoagulant\/#_ftn4\" name=\"_ftnref4\">[4]<\/a>).<\/span><\/p>\n<ul>\n<li><span style=\"font-size: 12pt;\">Chez les patients ayant un score HAS-BLED \u2265 3, prudence et examens r\u00e9guliers sont appropri\u00e9es, ainsi que les efforts \u00e0 corriger les facteurs de risque potentiellement r\u00e9versibles de saignement. Le HAS-BLED score ne doit pas \u00eatre utilis\u00e9 pour exclure les patients de la th\u00e9rapie OAC, mais permettre aux cliniciens de faire une \u00e9valuation \u00e9clair\u00e9e du risque h\u00e9morragique.<\/span><\/li>\n<li><span style=\"font-size: 12pt;\">Le risque h\u00e9morragique doit \u00eatre mis en balance avec le b\u00e9n\u00e9fice attendu par un traitement AVK bien suivi (r\u00e9duction d\u2019environ 60% de l\u2019incidence annuelle d\u2019embolie syst\u00e9mique), le b\u00e9n\u00e9fice \u00e9tant d\u2019autant plus fort que le risque primaire est \u00e9lev\u00e9. Si le score HAS-BLED est sup\u00e9rieur au score CHADS2, le risque h\u00e9morragique est sup\u00e9rieur au b\u00e9n\u00e9fice potentiel d\u2019une ACG orale (3). IPP \u00e0 envisager.<\/li>\n<li>Une ACO ne doit \u00eatre envisag\u00e9e qu\u2019apr\u00e8s un d\u00e9lai d\u00e9pendant de la s\u00e9v\u00e9rit\u00e9 de l\u2019AVC et une absence
